Some project administrators and individuals have developed their own consent forms for use within their DNA project or when recruiting family members to test for a personal project.

Blaine Bettinger has compiled some templates for beneficiary and consent forms which can be used under a Creative Commons Licence. The forms can be downloaded from this link in the Genetic Genealogy Tips and Techniques Group on Facebook.
